Okay I just do not get this!   I am not one for bashing another's workout but I had to post this and make an exception.   The workout below is for a 15 yr. old female athlete who is now 12 weeks post-op.  She just emailed this to me and said that this has been the basis of her sesions the last two weeks.   IT is a circuit workout to be completed three times through:

-Keiser resistace runner 
-side lunges across gym
-push press w/ med ball
-alt. leg pike med ball
-side kick balance and reach on squishy pad
-TRX hamstring
-TRX quads 
-8 count body builders

When I read this I immediately thought...Are you kidding me?  All this type of work is doing is breaking her down.   Where is the strength and power focus?  Where is any focus at all on deceleration?  I am sorry but side kick balance and reach on a 'squishy pad' has nothing to do with improving strength and stability around her knee joint.

So what would I do?  First of all since she is at twelve weeks I would perform a hop and stop test along with a basic leg strength rep test just to get an idea of what she currently can and cannot do.   I would then implement some of the following:

  • lunge and return w/variations
  • drop lunges/squats
  • box drop and stick (single leg)
  • single leg squat variations
  • single leg dead lifts
  • start fundamental skills such as shuffle to shuffle (focusing on deceleration mechanics); shuffle to jog; etc
  • single leg jump rope
  • bullet belt lateral marches and skips

Okay that is just a remedial list off of the top of my head but you get the idea.  I will post my meeting with this player in the next few days and let's see what the results of the evaluation show!

I appreciate your commends but please refrain a bit when making a statement such as "It's hard to believe that physical therapists and trainers out there still haven't figured it out yet." I am an ATC and CSCS and am a manager of a Physical Therapy/Athletic Training clinic. The reality of things is there are a lot of professionals (PT's, ATC's, Strength and Conditioning Coaches and Personal Trainers) that have yet to "figure it out". At the same time there are many of us out there that have. I work with many Physical Therapists and Athletic Trainers that provide the highest quality rehab that addresses all of what you speak of and in many cases have found Personal Trainers and Strength Coaches to all too often get caught up in performing exercises that have no application or purpose to an athletes rehabilitation. That all said your point is an important one for all of us as professionals to remember while at the same time being sure to avoid generalizations about professionals.

I do believe that both the original article and the comment above have much merit. But, you have to admit, that PT's and ATC's are "supposed" to be licensed professionals in order to create programs and progressions that will surely give the athlete the best chance at recovery. I agree that there may be more than one way to get that result, but there is no excuse for anyone to put players in programs that digress an athletes recovery or put them at greater risk for re-injury. Trainers/Strength Coaches should never take it upon themselves to do the work of a PT/ATC in post operative situations. Outside of a release from a doctor/PT, then we get to work...but we never get that chance if they cannot get to that point. All professionals need to check the validity of their programs.

I recently attended a lecture by Dr. Paul Juris on "the Biomechanics of Function", and it really hit home the need to analyze all our exercises and programs to rethink the REAL outcome of our process. Are our programs really setting our athletes up for success, or failure?
